Job summary

TEEMA Solutions Group is seeking a compassionate Physical Therapist (PT) to deliver high-quality rehabilitation services in a healthcare setting. The PT will evaluate patients, develop individualized treatment plans, and implement interventions aimed at restoring mobility and reducing pain.

The position requires collaboration with physicians and the interdisciplinary team, accurate documentation, patient and family education, and adherence to safety and regulatory standards across care settings.

Qualifications

• Active PT license in the state of practice.
• BLS/CPR certification preferred or required based on facility policy.
• Doctor of Physical Therapy (DPT) degree as required by state regulations.
• Experience in acute care, outpatient, inpatient rehabilitation, skilled nursing, home health, or specialty settings preferred.
• Strong clinical assessment, communication, documentation, and patient education skills.
• Ability to work independently and collaboratively within an interdisciplinary healthcare team.

Responsibilities

• Evaluate patients and develop individualized treatment plans based on functional needs and physician referrals.
• Provide therapeutic interventions to improve mobility, strength, balance, flexibility, coordination, pain management, and functional independence.
• Monitor patient progress, modify treatment plans as appropriate, and document all evaluations, treatments, and outcomes accurately.
• Educate patients, families, and caregivers regarding treatment plans, home exercise programs, injury prevention, and discharge recommendations.
• Collaborate with physicians and interdisciplinary healthcare teams to coordinate patient care and achieve optimal rehabilitation outcomes.
• Maintain accurate and timely clinical documentation in accordance with facility policies and regulatory requirements.
• Ensure compliance with patient safety, infection prevention, and quality assurance standards.
• Treat patients across assigned care settings and age groups as required by the facility.
• Perform other job-related duties as assigned.
