Job summary

TEEMA Solutions Group is seeking a compassionate Physical Therapist to deliver high-quality, patient-centered rehabilitation across acute and post-acute settings. The role includes evaluating patients, designing individualized plans, and applying evidence-based interventions to restore mobility and reduce pain.

The PT will collaborate with physicians and interdisciplinary teams, ensure thorough documentation, and educate patients and families on exercise programs and discharge planning.

Qualifications

• Active Physical Therapist license in the state of practice.
• Current BLS/CPR certification as required by facility policy.
• Doctor of Physical Therapy (DPT) degree or equivalent as required by state regulations.
• Experience in acute care, outpatient, inpatient rehab, SNF, home health, or specialty settings preferred.

Responsibilities

• Evaluate patients and develop individualized treatment plans based on functional needs and physician referrals.
• Provide therapeutic interventions to improve mobility, strength, balance, flexibility, coordination, pain management and independence.
• Monitor progress, modify plans as appropriate, and document evaluations, treatments and outcomes.
• Educate patients, families and caregivers on treatment plans, home exercises, injury prevention and discharge recommendations.
• Collaborate with physicians and interdisciplinary teams to coordinate care and optimize rehab outcomes.
• Maintain accurate clinical documentation in line with policies and regulatory requirements.
• Ensure compliance with patient safety, infection prevention and quality standards.
• Treat patients across assigned settings and age groups as required.
