Job summary

TEEMA Solutions Group is seeking a compassionate and skilled Physical Therapist (PT) to deliver evidence-based rehabilitation in a healthcare setting in Florida. The PT will examine patients, create individualized plans, and apply therapeutic interventions to restore mobility and reduce pain.

The role emphasizes collaboration with physicians, nurses, and interdisciplinary teams, with a focus on safe, effective care and thorough documentation to support outcomes and discharge planning.

Qualifications

• Active PT license in the state of practice.
• BLS/CPR certification preferred or required by facility policy.
• DPT degree or equivalent as required by state regulations.
• Experience in acute care, outpatient, inpatient rehab, SNF, home health, or specialty settings preferred.
• Strong clinical assessment, communication, documentation, and patient education skills.
• Ability to work independently and within an interdisciplinary team.

Responsibilities

• Evaluate patients and develop individualized treatment plans based on functional needs and physician referrals.
• Provide interventions to improve mobility, strength, balance, flexibility, coordination, pain management, and functional independence.
• Monitor patient progress, modify treatment plans as appropriate, and document evaluations, treatments, and outcomes.
• Educate patients, families, and caregivers regarding plans, home exercise programs, injury prevention, and discharge recommendations.
• Collaborate with physicians and care teams to coordinate patient care and achieve rehabilitation outcomes.
• Maintain accurate clinical documentation in line with policies and regulatory requirements.
• Ensure patient safety, infection prevention, and quality standards.
• Treat patients across assigned care settings and age groups as required by facility.
• Perform other job-related duties as assigned.
