Job summary

TEEMA Solutions Group is seeking a compassionate Physical Therapist (PT) to provide patient-centered rehabilitation services in a healthcare setting. The PT will evaluate patients, develop individualized treatment plans, and implement therapeutic interventions to restore mobility and function.

The role requires active PT licensure in the state of practice, a DPT or equivalent, and strong collaboration with physicians and care teams.

Qualifications

• Active Physical Therapist license in the state of practice.
• BLS/CPR certification as required by facility policy.
• Doctor of Physical Therapy (DPT) degree or equivalent per state regulations.
• Experience in acute care, outpatient, inpatient rehabilitation, skilled nursing, home health, or specialty settings.

Responsibilities

• Evaluate patients and develop individualized treatment plans based on functional needs and physician referrals.
• Provide therapeutic interventions to improve mobility, strength, balance, flexibility, coordination, pain management, and functional independence.
• Monitor progress, adjust treatment plans, and document evaluations, treatments, and outcomes.
• Educate patients, families, and caregivers on treatment plans, home exercise programs, discharge recommendations.
• Collaborate with physicians and interdisciplinary healthcare teams to coordinate patient care.
• Maintain accurate clinical documentation in accordance with policies and regulatory requirements.
• Treat patients across assigned care settings and age groups as required.
• Perform other job-related duties as assigned.
