Job summary

TEEMA Solutions Group is seeking a compassionate Physical Therapist (PT) to provide high-quality rehabilitation in a healthcare setting. You will evaluate patients, create individualized plans, apply therapeutic interventions, and monitor progress to restore mobility and reduce pain.

Collaboration with physicians, nurses, and interdisciplinary teams is essential to ensure safe, evidence-based care. Qualified candidates hold an active PT license, DPT, and BLS/CPR as required by facility policy,

Qualifications

Active Physical Therapist license in the state of practice.
BLS/CPR certification as per facility policy.
DPT degree or equivalent per state requirements.
Experience in acute care, outpatient, inpatient rehab, SNF, home health, or specialty settings preferred.
Strong clinical assessment, communication, documentation, and patient education skills.

Responsibilities

Evaluate patients and develop individualized treatment plans.
Provide interventions to improve mobility, strength, balance, and function.
Monitor progress and modify plans; document evaluations and outcomes.
Educate patients, families, and caregivers on home programs and discharge plans.
Collaborate with physicians and care teams to coordinate patient care.
Maintain accurate clinical documentation per policy and regulation.
