Cherrydale Post Acute in Greenville, SC is seeking a PRN Physical Therapist to evaluate and treat residents, improve mobility, and collaborate with an interdisciplinary rehab team to provide patient-centered care.

This flexible PRN role offers competitive pay of $40-$50 per hour, opportunities for advancement, and a supportive culture under new leadership.

Qualifications

• Licensed to practice Physical Therapy in South Carolina.
• Bachelor's degree in Physical Therapy; advanced degree preferred.
• Experience in a skilled nursing or rehab setting preferred.
• Familiarity with PCC and Casamba.

Responsibilities

• Perform patient evaluations and develop individualized treatment plans.
• Provide therapy to improve mobility, strength, balance, and functional independence.
• Monitor patient progress and adjust treatment plans as needed.
• Maintain accurate and timely clinical documentation.
• Communicate effectively with physicians, residents, families, and care team members.
• Participate in care planning and discharge planning.
• Conduct staff education and in-service training.
• Supervise PTAs, aides, and students as needed.

Position Summary

As a Physical Therapist (PT), you will evaluate and treat residents to improve mobility, strength, and overall function. You will work closely with an interdisciplinary team to deliver personalized, patient-centered care.

• Active, unencumbered Physical Therapist license in South Carolina (required)
• Experience in a skilled nursing or rehab setting preferred
• Familiarity with PointClickCare (PCC) and Casamba preferred
• Strong clinical, communication, and leadership skills
• Commitment to patient-centered care
