Job summary

Chandler Creek Post Acute in Greer, SC is seeking a dedicated Physical Therapist to join our full-time rehab team. You will evaluate, develop, and implement personalized treatment plans to help residents improve mobility and independence.

The role features a day shift, competitive pay of $45-$50/hour, and a comprehensive benefits package including medical, dental, vision, and life insurance, PTO, and a 401(k) with company match. Prior long-term care experience is preferred.

Qualifications

• Bachelor's degree required; advanced degree preferred
• Active, unencumbered Physical Therapist license in South Carolina
• Experience in a long-term care or rehab setting preferred
• Familiarity with PointClickCare and Casamba preferred
• Strong communication and clinical decision-making skills
• Passion for patient-centered care

Responsibilities

• Conduct patient evaluations and develop customized treatment plans
• Provide therapeutic interventions to improve strength, mobility, and function
• Monitor patient progress and adjust treatment plans as needed
• Educate residents and families on therapy techniques and goals
• Maintain accurate and timely clinical documentation
• Collaborate with interdisciplinary teams to ensure optimal patient outcomes
