Job summary

Simpsonville Post Acute in Simpsonville, SC is seeking a licensed Physical Therapist (PT) to provide high-quality, patient-centered therapy on a PRN basis. The role focuses on helping residents improve mobility, strength, and independence as part of an interdisciplinary care team.

You will evaluate residents, develop treatment plans, provide therapy per physician orders, document progress, and educate residents and families.

Qualifications

• Degree in Physical Therapy (Bachelor's required; Master's or Doctorate preferred).
• Active SC Physical Therapist license in good standing.
• Experience in skilled nursing or rehabilitation setting preferred.
• Familiarity with PointClickCare and Casamba preferred.
• Strong clinical, documentation, and communication skills.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Provide skilled therapy services per physician orders and care plans.
• Document evaluations, treatments, and progress notes in compliance with regulations.
• Communicate effectively with physicians, nursing staff, and care team members.
• Supervise PTAs, aides, and students in accordance with state practice acts.
• Participate in care planning and interdisciplinary meetings.
• Educate residents, families, and caregivers on therapy goals and discharge plans.
• Conduct staff training and in-service education.
• Maintain compliance with all state, federal, and payer guidelines.
