Job summary

Continuum Therapy Partners, LLC is seeking a licensed Physical Therapist to join our La Crescent, MN team. This PRN role supports our therapist-led care model and collaborates with a leadership group that values clinical judgment.

Candidates must hold a Doctor of Physical Therapy (DPT), a current MN PT license, and be eligible to pass the NPTE. One year of PT experience is preferred, with a focus on evaluating patients and developing individualized plans.

Responsibilities

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
• Documents patient progress and adjusts plans of care based on clinical outcomes.

Physical Therapist Requirements & Responsibilities

• Doctor of Physical Therapy (DPT) degree required
• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE)
• 1 year of previous experience as a Physical Therapist preferred
• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines
• Documents patient progress and adjusts plans of care based on clinical outcomes
• Educates patients and families on home exercise programs and injury prevention
