Job summary

Continuum Therapy Partners, LLC is seeking a Physical Therapist to provide patient-centered care at the Zumbrota location. A Doctor of Physical Therapy (DPT) is required, with current NPTE licensure.

PRN schedule available and prior experience preferred for effective treatment planning and patient education. Responsibilities include evaluating and developing individualized treatment plans, coordinating with other disciplines, and documenting progress to adjust care.

Qualifications

• DPT required and NPTE license must be current or in progress.
• Active state PT license; NPTE must be passed.
• 1 year of clinical PT experience preferred.

Responsibilities

• Documents patient progress and adjusts plans of care based on clinical outcomes.

Physical Therapist Requirements & Responsibilities

• Doctor of Physical Therapy (DPT) degree required
• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE)
• 1 year of previous experience as a Physical Therapist preferred
• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines
• Documents patient progress and adjusts plans of care based on clinical outcomes
• Educates patients and families on home exercise programs and injury prevention
