Job summary

Continuum Therapy Partners, LLC in Saint Charles, MN is seeking a Physical Therapist to provide patient-centered care on a PRN basis at Whitewater Health Services.

You will evaluate and implement treatment plans, coordinate with other disciplines, and educate patients and families on home exercises and prevention.

Qualifications

• Doctor of Physical Therapy (DPT) degree required.
• Current, unencumbered state Physical Therapy License; must pass the NPTE.
• 1 year of previous experience as a Physical Therapist preferred.

Responsibilities

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
• Documents patient progress and adjusts plans of care based on clinical outcomes.
• Educates patients and families on home exercise programs and injury prevention.
