Job summary

Continuum Therapy Partners, LLC is seeking a Physical Therapist to join our Browns Valley, MN location on a PRN schedule. The candidate must hold a Doctor of Physical Therapy (DPT) degree and a current, unrestricted state Physical Therapy License with NPTE eligibility.

A minimum of one year of PT experience is preferred, with responsibilities including assessment, plan development, and coordination with other disciplines to ensure optimal patient outcomes.

Physical Therapist Requirements & Responsibilities

• Doctor of Physical Therapy (DPT) degree required
• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE)
• 1 year of previous experience as a Physical Therapist preferred
• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines
• Documents patient progress and adjusts plans of care based on clinical outcomes
• Educates patients and families on home exercise programs and injury prevention
