Continuum Therapy Partners, LLC is seeking a Physical Therapist in Northfield, MN for a PRN schedule at Three Links Care Center. A Doctor of Physical Therapy (DPT) is required, with an active state license and NPTE completion. Prior PT experience is preferred to assess and implement individualized care plans.

The role emphasizes evaluating progress, documenting outcomes, and educating patients and families on home exercises and injury prevention to promote recovery and mobility.
