Job summary

Pivot Home Health is seeking a Physical Therapist (PT) to provide home-based therapy for pediatric and adult clients in Colorado. You will assess, plan, and implement individualized interventions to improve function and independence in clients’ homes.

Join a clinician-first team with autonomy, flexible scheduling, and iPad-enabled documentation. We welcome new grads and relocating clinicians to apply for FT or PT roles in a supportive, growth-oriented environment.

Qualifications

• Active Colorado Physical Therapist (PT) license.
• Current CPR certification.
• New graduates and relocating clinicians encouraged to apply.
• Excellent communication and collaboration skills.

Responsibilities

• Provide pediatric and adult home-based therapy to build skills and independence.
• Develop and implement individualized treatment plans for home health clients.
• Document progress using electronic records on iPad.
• Collaborate with families and care teams to support goals.

Physical Therapist (PT) - Pediatric Home Health

Region: Aurora, CO
We are hiring throughout the Front Range and encourage you to still apply if interested in other locations!
Employment Type: full-time or part-time (FT is 20 visits/week)
Compensation: $80-$90+ per regular visit
Caseload: Adults and/or pediatrics
