Job summary

CommuniCare Health Services seeks a Physical Therapist (PT) for Berkeley Springs Healthcare Center. This in-house team member position enables you to directly impact residents' rehabilitation through patient-centered therapy.

You will evaluate charts, develop treatment plans, supervise PTAs, and collaborate with an interdisciplinary rehab team. The role offers potential to serve across multiple centers and supports ongoing professional growth.

Qualifications

• Bachelor's, Master’s, or Doctorate in Physical Therapy from an accredited program.
• Valid or pending State Physical Therapy License.

Responsibilities

• Evaluate, Review, and Screen patient’s/resident’s medical charts, evaluations under physician order and develop a treatment plan in accordance with patient’s needs.
• Implement treatment plan through direct treatment, as well as education of the plan of care to all team members, including providing necessary supervision for PTAs.
• Attend/Lead interdisciplinary meetings to relay clinical findings and make recommendations to other members of rehab team.

Qualifications:
