BAYADA Home Health Care is seeking an experienced Physical Therapist (PT) for full-time, part-time, or per diem opportunities. The role involves performing home health visits for our Fayetteville office, located in Fayetteville, NC. This office services adult and geriatric clients across territories in Bladen, Cumberland, Harnett, and Hoke counties.

As a home care Physical Therapist, you will be a vital part of a multidisciplinary healthcare team providing rehabilitative care and skilled nursing, enabling clients to remain at home.

Responsibilities:

1. Make home visits to clients in designated areas.
2. Perform diagnostic tests and measurements related to mobility, strength, balance, and safety.
3. Develop and implement individualized care plans, including manual exercises, gait training, and balance interventions.
4. Assess and revise care plans regularly, collaborating with nursing staff as needed.
5. Educate clients and families on rehabilitative activities to promote health and independence.
6. Document observations and interventions accurately using electronic medical records.
7. Implement wound care interventions using evidence-based dressings and topical agents.

Qualifications:

1. Current North Carolina Physical Therapist (PT) license.
2. Graduation from an accredited program by the American Physical Therapy Association or equivalent.
3. Ability to work independently and manage time effectively.
4. Strong interpersonal, organizational, and problem-solving skills.
5. Proficiency with computers; experience with EMR preferred.
