BAYADA Home Health Care is seeking an experienced Physical Therapist (PT) for full-time, part-time, or per diem roles providing home health visits for our Fayetteville, NC office. This office serves adult and geriatric clients across Bladen, Cumberland, Harnett, Moore, and Hoke counties.

As a home care Physical Therapist, you will be a key member of a multidisciplinary team delivering rehabilitative and skilled nursing care to enable clients to remain at home.

Responsibilities include:

1. Making home visits within designated territories.
2. Performing diagnostic tests, including mobility, strength, balance, and safety assessments.
3. Developing and updating personalized care plans with interventions like manual exercises, gait training, and balance activities.
4. Collaborating with nursing staff on the multidisciplinary care plan.
5. Educating clients and families on rehabilitative activities to promote health and independence.
6. Documenting care activities using electronic tablets.
7. Applying wound care interventions using evidence-based dressings and topical agents.

Qualifications:

1. Current North Carolina PT license.
2. Graduation from an APTA-approved program or equivalent.
3. Ability to work independently and manage time effectively.
4. Strong interpersonal, organizational, and problem-solving skills.
5. Proficiency with computers; experience with EMR preferred.
