BAYADA Home Health Care is seeking an experienced Physical Therapist (PT) for full-time, part-time, or per diem home health visits in our Fayetteville, NC office. This role services adult and geriatric clients across territories in Bladen, Cumberland, Harnett, Moore, and Hoke counties.

As a home care Physical Therapist, you will be an essential part of a multidisciplinary team providing rehabilitative and skilled nursing care, enabling clients to remain at home.

Responsibilities:

1. Make home visits to clients in designated areas.
2. Perform diagnostic tests and measurements, including mobility, joint range, stability, ambulation patterns, strength, endurance, and safety assessments.
3. Develop and adjust individualized care plans, including manual exercises, gait training, and balance interventions.
4. Assess and revise care plans, collaborating with nursing staff as needed.
5. Educate clients and families on rehabilitative activities to promote health and independence.
6. Document observations and interventions accurately using electronic tablets.
7. Implement wound prevention and repair interventions using evidence-based dressings and topical agents.

Qualifications:

• Current North Carolina Physical Therapist (PT) license.
• Graduation from an accredited program approved by the American Physical Therapy Association.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Proficiency with computers; experience with electronic medical records preferred.
