Empath Home Health, a member of Empath Health, is seeking a compassionate PRN Home Health Physical Therapist to provide in-home care in south Pinellas County including St. Petersburg and Clearwater.

You will perform assessments, develop plans of care, and deliver skilled PT services with family education. Flexible scheduling with 5-15+ visits per week, weekend/holiday rotations, and potential growth to full-time as the market grows.

Formación

• Active Florida PT license
• Minimum of 1 year experience as a licensed PT
• Home health care experience preferred
• Strong assessment and patient education skills
• Willingness to participate in weekend and holiday rotations

• Develop, initiate, and revise individualized plans of care
• Deliver skilled physical therapy services per the plan of care
• Counsel and educate patients and families on disease processes and self-care

Locations:

Home health visits available in SouthPinellas county (St. Petersburg, Pinellas Park, Seminole, Gulfport, South Pasadena, Clearwater, Largo). It is very flexible - you choose when & where you want to work!

If desired, visits may also be available in surrounding counties!

Schedule

PRN; 5-15+ home health visits per week. Required weekend/holiday rotations.

What you’ll Do

• Perform comprehensive patient assessments, including OASIS assessments at required intervals
• Develop, initiate, and revise individualized plans of care as patient needs evolve
• Deliver skilled physical therapy services and treatments as outlined in the plan of care
• Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies
• Collaborate with the interdisciplinary team to ensure seamless, holistic care

What You’ll Need

• Active Florida PT license
• Minimum of 1 year experience as a licensed Physical Therapist (PT).
• Home health care experience strongly preferred.
