Job summary

Empath Home Health, a member of Empath Health, seeks a compassionate PRN Home Health Physical Therapist (PT) to provide exceptional care to patients in Arcadia, FL and surrounding areas. The role offers flexible, PRN scheduling with 5-10+ home visits per week and opportunities for growth within the organization.

Requirements include an active Florida PT license and a minimum of 1 year of licensed PT experience; home health experience is preferred.

Qualifications

• Willingness to participate in weekend and holiday on-call rotations.

Responsibilities

• Perform comprehensive patient assessments, including OASIS assessments at required intervals.
• Develop, initiate, and revise individualized plans of care as patient needs evolve.
• Deliver skilled physical therapy services and treatments as outlined in the plan of care.
• Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies.
• Collaborate with the interdisciplinary team to ensure seamless, holistic care.

Schedule : PRN; 5-10+ home health visits available per week

What you’ll Do
