Empath Home Health in Florida is seeking a compassionate PRN Home Health Physical Therapist to provide individualized care in south Pinellas County (St. Petersburg, Clearwater, and surrounding areas).

You will perform assessments, initiate plans of care, and deliver PT services in patients' homes. This role offers flexible scheduling with 5-15+ visits per week, required weekend/holiday rotations, and mileage reimbursement.

Qualificações

• Active Florida PT license.
• Minimum 1 year of experience as a licensed Physical Therapist.
• Home health care experience preferred.
• Strong assessment and patient education skills.
• Willingness to participate in weekend and holiday rotations.

Responsabilidades

• Perform comprehensive patient assessments, including OASIS assessments at required intervals.
• Develop, initiate, and revise individualized plans of care as patient needs evolve.
• Deliver skilled physical therapy services and treatments as outlined in the plan of care.
• Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies.
• Collaborate with the interdisciplinary team to ensure seamless, holistic care.
