Job summary

Empath Home Health in Fort Myers, FL is seeking a compassionate PRN Home Health Physical Therapist (PT) to deliver high‑quality, patient‑centered care in patients' homes across Fort Myers and surrounding areas.

You will perform comprehensive patient assessments, develop and revise plans of care, deliver skilled PT services, and educate patients and families on disease processes and self‑care. Flexible scheduling and weekend/holiday on‑call rotations are available to fit your needs.

Qualifications

• Active Florida PT license.
• Minimum of 1 year experience as a licensed Physical Therapist (PT).
• Home‑health care experience strongly preferred.
• Strong assessment, critical‑thinking, and patient education skills.
• Willing to participate in weekend and holiday on‑call rotations.

Responsibilities

• Perform comprehensive patient assessments, including OASIS assessments at required intervals.
• Develop, initiate, and revise individualized plans of care as patient needs evolve.
• Deliver skilled physical therapy services and treatments as outlined in the plan of care.
• Counsel and educate patients and families on disease processes, self‑care techniques, and prevention strategies.
• Collaborate with the interdisciplinary team to ensure seamless, holistic care.

Home health visits in the Fort Myers area / Lee county (including Fort Myers, Cape Coral, Lehigh Acres, Estero, Bonita Springs , and surrounding areas) . It is flexible - you choose when & where you want to work!

Schedule :

PRN; 5-10+ home health visits available per week

What you’ll Do
