Job summary

Empath Home Health in Fort Myers, FL is seeking a compassionate PRN Home Health Physical Therapist (PT) to deliver patient-centered care in patients' homes across Fort Myers and surrounding areas.

You’ll perform home health evaluations, develop and revise plans of care, provide skilled PT, educate patients and families, and collaborate with an interdisciplinary team. Florida license and 1+ year home health experience preferred; available weekends/holidays.

Qualifications

• Active Florida PT license.
• Minimum 1 year experience as licensed PT.
• Home health care experience preferred.
• Strong assessment, critical thinking, and patient education skills.
• Willingness to participate in weekend and holiday on-call rotations.

Responsibilities

• Perform comprehensive patient assessments including OASIS at required intervals.
• Develop, initiate, and revise individualized plans of care as patient needs evolve.
• Deliver skilled physical therapy services and treatments per plan of care.
• Counsel and educate patients and families on disease processes and self-care.
• Collaborate with the interdisciplinary team to ensure seamless, holistic care.

Locations: Home health visits in the Fort Myers area / Lee county (including Fort Myers, Cape Coral, Lehigh Acres, Estero, Bonita Springs , and surrounding areas) . It is flexible - you choose when & where you want to work!

Schedule : PRN; 5-10+ home health visits available per week

What you’ll Do

• Perform comprehensive patient assessments, including OASIS assessments at required intervals
• Develop, initiate, and revise individualized plans of care as patient needs evolve
• Deliver skilled occupational therapy services and treatments as outlined in the plan of care
• Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies
• Collaborate with the interdisciplinary team to ensure seamless, holistic care
