Job summary

Empath Home Health is seeking a compassionate PRN Physical Therapy Assistant to deliver in-home therapy across east Pasco county, including Wesley Chapel, Odessa, Zephyrhills, Land O’ Lakes, and surrounding areas. Work 5–10+ visits per week with potential to grow to full-time as demand grows.

You’ll provide direct PT assistant services under a Physical Therapist, support patient education, and coordinate with an interdisciplinary team for comprehensive home‑based care.

Qualifications

• Active Florida PTA license.
• Minimum of 1 year experience as a licensed PTA.
• Home health care experience strongly preferred.
• Strong assessment, critical thinking and patient education skills.

Responsibilities

• Provide physical therapy services to patients in their homes under the supervision of a Physical Therapist in accordance with the plan of care and agency policies.
• Assist the Physical Therapist in evaluating equipment needs to increase functional level.
• Educate patients and families on self-care and techniques to improve or maintain functioning.
• Collaborate with the interdisciplinary team to ensure holistic, seamless care.

Locations: Home health visits in east Pasco county (Wesley Chapel, Odessa, Land O' Lakes, Dade City, Zephyrhills, and surrounding areas)

Schedule : PRN role; 5-10+ home health visits per week. Full-time may be available as well!

What You’ll Do

• Provide physical therapy services to patients in compliance with agency policies and procedures, federal and state regulations, following the plan of care under the supervision of a Physical Therapist.
• Assist the Physical Therapist in evaluating equipment needs of patient to increase functional level.
• Participates in the education of patient and family on methods of self-care and techniques to improve or maintain patient’s level of functioning.
• Collaborate with the interdisciplinary team to ensure seamless, holistic care
