Job summary

Empath Home Health in Florida is seeking a compassionate PRN Home Health Physical Therapist to join the Home Health team serving Hernando County and surrounding areas.

You will perform patient assessments, develop and revise plans of care, deliver PT services at patients' homes, and educate patients and families. Weekend and holiday on-call rotations are required, with potential expansion to part-time or full-time as the market grows.

Qualifications

• Active Florida PT license.
• Minimum of 1 year experience as a licensed Physical Therapist (PT).
• Home health care experience strongly preferred.
• Strong assessment, critical thinking, and patient education skills.
• Willingness to participate in weekend and holiday on–call rotations.

Responsibilities

• Perform comprehensive patient assessments, including OASIS assessments at required intervals.
• Develop, initiate, and revise individualized plans of care as patient needs evolve.
• Deliver skilled physical therapy services and treatments as outlined in the plan of care.
• Counsel and educate patients and families on disease processes, self‑care techniques, and prevention strategies.
• Collaborate with the interdisciplinary team to ensure seamless, holistic care.

Empath Home Health, a member of Empath Health, is seeking a compassionate PRN Home Health PT (Physical Therapist) to work with our Home Health team to deliver individualized care to patients in their homes in Hernando County (including Spring Hill, Weeki Wachee, Brooksville, Ridge Manor, and surrounding areas).

Empath Home Health provides skilled nursing and therapy services to help patients recover and maintain independence at home, along with personal care support that assists with daily needs as part of a seamless continuum of care.

Locations: Home health visits throughout Hernando County (including Spring Hill, Weeki Wachee, Brooksville, Ridge Manor, and surrounding areas)

Schedule: 5-10+ home health visits per week. Required weekend/holiday on-call rotations. Role may expand to part-time or full-time as we grow!
