Job summary

Empath Home Health, a member of Empath Health, is seeking a compassionate PRN Home Health PT to deliver skilled physical therapy in east Pasco county and surrounding areas. You will assess patients, develop plans of care, provide treatments, educate families, and collaborate with the interdisciplinary team to ensure seamless, holistic care in the patients' homes.

Active Florida PT license and minimum 1 year of licensed PT experience required; home health experience preferred.

Qualifications

• Active Florida PT license.
• Minimum of 1 year experience as a licensed PT.
• Home health care experience strongly preferred.
• Strong assessment, critical thinking, and patient education skills.
• Willingness to participate in weekend/holiday rotations.

Responsibilities

• Perform comprehensive patient assessments, including OASIS assessments at required intervals.
• Develop, initiate, and revise individualized plans of care as patient needs evolve.
• Deliver skilled physical therapy services and treatments as outlined in the plan of care.
• Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies.
• Collaborate with the interdisciplinary team to ensure seamless, holistic care.

Locations: Home health visits ineast Pasco county (Dade City, Zephyrhills, Wesley Chapel, and surrounding areas)

If desired, visits may also be available in west Pasco, Hernando, and/or Hillsborough counties.

Schedule : PRN; 5-15+ home health visits per week. Required weekend/holiday rotations.

What You’ll Need

• Active Florida PT license
