Job summary

Empath Home Health in Manatee County, FL is seeking a compassionate PRN Home Health Physical Therapist to deliver skilled therapy in patients’ homes across Bradenton, Lakewood Ranch, Anna Maria Island, Ellenton, and surrounding areas. The role requires an active Florida PT license and at least 1 year of PT experience, with home health experience preferred.

You will perform comprehensive assessments (including OASIS as required), develop and adjust plans of care, educate patients and families,

Responsibilities

• Perform comprehensive patient assessments, including OASIS assessments at required intervals
• Develop, initiate, and revise individualized plans of care as patient needs evolve
• Deliver skilled physical therapy services and treatments as outlined in the plan of care
• Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies
• Collaborate with the interdisciplinary team to ensure seamless, holistic care

Empath Home Health , a member of Empath Health is seeking a compassionate PRN Home Health PT Physical Therapist to work with us to provide exceptional and individualized care in Manatee county (Bradenton, Lakewood Ranch, and surrounding areas).

Locations: Home health visits in Manatee county (including Bradenton, Lakewood Ranch, Anna Maria Island, Ellenton, Palmetto , and surrounding areas).

Visits may also be available in Sarasota and/or Hillsborough counties if desired.

Schedule : PRN; 5-10+ home health visits per week. Required weekend/holiday on-call rotations.

What You’ll Need

• Active Florida PT license
