Job summary

Empath Home Health in Florida is seeking a compassionate PRN Occupational Therapist (OT) to provide skilled home health OT services across Hernando County (Brooksville, Spring Hill, Weeki Wachee and surrounding areas). Ideal candidates hold an active Florida OT license and have at least 1 year of OT experience, preferably in home health, with strong assessment and patient education skills.

Responsibilities

• Perform comprehensive patient assessments, including OASIS assessments at required intervals
• Develop, initiate, and revise individualized plans of care as patient needs evolve
• Deliver skilled occupational therapy services and treatments as outlined in the plan of care
• Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies
• Collaborate with the interdisciplinary team to ensure seamless, holistic care

Locations: Home health visits in Hernando county (Brooksville, Spring Hill, Weeki Wachee, and surrounding areas).

If desired, visits may also be available in Pasco or Hillsborough counties.

Schedule : PRN; 5-15+ home health visits per week.

What You’ll Need

• Active Florida OT license
