Job summary

Empath Home Health, part of Empath Health, is seeking a compassionate full-time Home Health PT Physical Therapist to provide individualized care across Hendry county, including Clewiston, LaBelle and Lehigh Acres. The role includes initial and ongoing assessments, plan-of-care development with the physician, and collaboration with the care team to achieve mobility goals.

Requirements include an active Florida PT license and at least 1 year of PT experience; home health experience is preferred.

Qualifications

• Active Florida PT license.
• Minimum 1 year experience as a licensed Physical Therapist (PT).
• Home health care experience preferred.
• Strong assessment, critical thinking, and patient education skills.

Responsibilities

• Provide physical therapy services to patients as prescribed by a physician in compliance with agency policies and regulations.
• Perform initial and ongoing assessments, including OASIS when appropriate, to determine function and daily living activities.
• Develop and revise plans of care with the physician and care team to achieve mobility goals.
• Report patient responses and condition changes to the physician and agency representative.

Locations: Home health visits in Hendry county (including Clewiston, LaBelle, Lehigh Acres, Moore Haven, Buckingham , and surrounding areas)

Schedule : Full-Time; M-F 40 hours with required weekend/holiday rotations as needed

What you’ll Do

• Provide physical therapy services to patient as prescribed by a physician in compliance with agency policies and procedures and federal and state regulations.
• Perform initial and ongoing assessments, including OASIS assessments at appropriate time points, to determine level of functioning and patient’s ability to perform activities of daily living.
• Develop and revise plans of care, in consultation with the physician and other care team members, which will enable patient to achieve realistic goals of mobility.
• Reports to the physician and designated agency representative patient’s reaction to treatment and any changes in the patient’s condition.

What You’ll Need

• Active Florida PT license
• Minimum of 1 year experience as a licensed Physical Therapist (PT).
• Home health care experience strongly preferred.
