Job summary

Transition Home Healthcare is an elite home health agency serving Houston, TX and surrounding areas since 2010. We are growing and looking for a dedicated Physical Therapist (PT) to join our team.

Shifts available: PRN & FT; service areas with openings: Northeast, Northwest, Southwest. Responsibilities include facilitating rehabilitation, implementing care plans, coordinating with the home health team, educating patients and families, and documenting notes.

Qualifications

• Licensed Physical Therapist in Texas.
• Home health experience preferred.
• Knowledge of home health regulations and compliance.
• Experience with HCHB EMR (preferred).
• Able to handle physical workload.
• Good driving record.
• Excellent communication and people skills to interact with patients and teams.

Responsibilities

• Facilitate rehabilitation of patients with emotional, physical or mental disabilities.

• Prepare timely, complete, and accurate clinical notes.
