Job summary

Transition Home Healthcare in Houston, TX is seeking a dedicated Physical Therapist Assistant (PTA) to join our growing team. Shifts available PRN and Full Time; coverage across Northwest, Northeast, Southeast, Southwest areas.

You will assist in rehabilitation, implement care plans, educate patients, and coordinate with the home health team while maintaining accurate clinical notes. Prior home health experience preferred; valid Texas PTA license required.

Qualifications

• Texas PTA license is required.
• Home health experience preferred.
• Able to handle physical workload.
• Excellent communication and people skills.

Responsibilities

• Assist in the rehabilitation of patients with emotional, physical or mental disabilities.

• Assist in the facilitation of rehabilitation of patients with emotional, physical or mental disabilities.
• Assist in the implementation of physical therapy care plans.
• Assist with the coordination of care with the home healthcare team.
• Educate patients and family members about health conditions.
• Prepare timely, complete, and accurate clinical notes.
• Communicate effectively with patients, caregivers, healthcare providers, office support, and other care team members.

Qualifications

• Assistant Physical Therapist in the state of Texas.
• Home health experience preferred.
• Knowledge of home health regulations and general healthcare compliance.
• Experience with HCHB EMR preferred.
• Able to handle physical workload.
• Good driving record.
• Excellent communication and people skills to effectively interact with patients, families, and healthcare team members.
