Job summary

Transition Home Healthcare in Houston, TX is seeking a dedicated Physical Therapist Assistant (PTA) to join our home health team. We offer both PRN and full-time shifts to cover Northeast, Northwest, and Southeast service areas.

The PTA will assist in rehabilitation, implement care plans, coordinate with the home health team, educate patients and families, and maintain timely, accurate clinical notes. We provide competitive pay, comprehensive benefits, and a supportive work environment.

Qualifications

• Texas PTA license required.
• Home health experience preferred.
• Knowledge of home health regulations and healthcare compliance.
• Experience with HCHB EMR (preferred).
• Able to handle physical workload.
• Good driving record.
• Excellent communication and people skills.

Responsibilities

• Assist in the facilitation of rehabilitation of patients with emotional, physical or mental disabilities.
• Assist in the implementation of physical therapy care plans.
• Assist with the coordination of care with the home healthcare team.
• Educate patients and family members about health conditions.
• Prepare timely, complete, and accurate clinical notes.
• Communicate effectively with patients, caregivers, healthcare providers, office support, and other care team members.
