Job summary

Transition Home Healthcare is seeking a dedicated Physical Therapist to serve Houston, TX and surrounding areas. The role offers FT and PRN shifts, with openings across Northeast, Northwest, and Southwest service areas.

In this position, you will implement PT care plans, educate patients and families, prepare accurate clinical notes, and collaborate with the home health team to coordinate care. Strong interpersonal skills and a valid TX PT license are essential.

Qualifications

• Must be a Texas-licensed Physical Therapist.
• Home health experience is highly preferred.
• Knowledge of home health regulations and healthcare compliance.
• Experience with HCHB EMR is preferred.
• Able to handle physical workload.

Responsibilities

• Facilitate the rehabilitation of patients with emotional, physical or mental disabilities.

Qualifications

• Physical Therapist in the state of Texas.
• Home health experience highly preferred.
