Job summary

Transition Home Healthcare is a Houston, TX-based home health agency seeking a dedicated Physical Therapist (PT). PRN and FT shifts available. Service areas include Northeast, Northwest, and Southwest.

You will implement care plans and coordinate with the home health team to deliver high-quality therapy in patients’ homes. Qualifications include a Texas PT license and preferred home health experience; strong communication and documentation skills are essential.

Qualifications

• Licensed Physical Therapist in Texas
• Home health experience preferred
• Knowledge of home health regulations and compliance
• Experience with HCHB EMR preferred

Responsibilities

• Facilitate the rehabilitation of patients with emotional, physical or mental disabilities.
• Implement physical therapy care plans.
• Coordinate care with the home healthcare team.
• Educate patients and family members about health conditions.
• Prepare timely, complete, and accurate clinical notes.
• Communicate effectively with patients, caregivers, healthcare providers, office support, and other care team members.

Qualifications

• Physical Therapist in the state of Texas.
• Home health experience highly preferred.
• Knowledge of home health regulations and general healthcare compliance.
• Experience with HCHB EMR. (preferred)
• Able to handle physical workload.
• Good driving record.
• Excellent communication and people skills to effectively interact with patients, families, and healthcare team members.
