Job summary

Transition Home Healthcare in Houston, TX is seeking a dedicated Physical Therapist (PT) to support patients in their homes. Shifts available PRN and FT; you will develop and implement rehabilitation plans, coordinate with the home health team, and educate patients and families. Texas license is required; home health experience is preferred.

Strong communication and a valid driving record are essential. We offer mileage reimbursement, scrubs, PTO, medical, dental, vision, life and disability

Qualifications

• Licensed Physical Therapist in Texas.
• Home health experience preferred.
• Knowledge of home health regulations and healthcare compliance.
• Experience with HCHB EMR is a plus.
• Ability to handle physical workload and drive safely.
• Good driving record and strong communication with patients and team.

Responsibilities

• Facilitate the rehabilitation of patients with emotional, physical or mental disabilities.
• Implement physical therapy care plans.
• Coordinate care with the home healthcare team.
• Educate patients and family members about health conditions.
• Prepare timely, complete, and accurate clinical notes.
• Communicate effectively with patients, caregivers, healthcare providers, office support, and other care team members.

Qualifications

• Physical Therapist in the state of Texas.
• Home health experience highly preferred.
• Knowledge of home health regulations and general healthcare compliance.
• Experience with HCHB EMR. (preferred but not required)
• Able to handle physical workload.
• Good driving record.
• Excellent communication and people skills to effectively interact with patients, families, and healthcare team members.
