Job summary

Transition Home Healthcare in Houston, TX is seeking a dedicated Physical Therapist Assistant (PTA) to join our elite home health team. Shifts available: PRN and Full Time, covering Northwest, Northeast, Southeast, and Southwest service areas.

Qualifications

• Assistant Physical Therapist in the state of Texas.
• Home health experience preferred.
• Knowledge of home health regulations and general healthcare compliance.
• Experience with HCHB EMR preferred.
• Able to handle physical workload.
• Good driving record.
• Excellent communication and people skills to effectively interact with patients, families, and healthcare team members.

Responsibilities

• Assist in the facilitation of rehabilitation of patients with emotional, physical or mental disabilities.
• Assist in the implementation of physical therapy care plans.
• Assist with the coordination of care with the home healthcare team.
• Educate patients and family members about health conditions.
• Prepare timely, complete, and accurate clinical notes.
• Communicate effectively with patients, caregivers, healthcare providers, office support, and other care team members.
