Job summary

Transition Home Healthcare in Houston, TX seeks a licensed Assistant Physical Therapist to support rehabilitation in home health settings. This PTA will assist in implementing care plans, educating patients and families, and coordinating with the care team.

Shifts include PRN and full-time, serving Northwest, Northeast, Southeast, and Southwest areas. The ideal candidate has TX PTA licensure, strong communication, and the ability to manage clinical notes and documentation in a home health

Qualifications

• Texas PTA license required.
• Home health experience preferred.
• Knowledge of home health regulations and compliance.
• Experience with HCHB EMR preferred.

Responsibilities

• Assist in the facilitation of rehabilitation of patients with emotional, physical or mental disabilities.
• Assist in the implementation of physical therapy care plans.
• Assist with the coordination of care with the home healthcare team.
• Educate patients and family members about health conditions.
• Prepare timely, complete, and accurate clinical notes.
• Communicate effectively with patients, caregivers, healthcare providers, office support, and other care team members.
