Job summary

Pride Health is hiring a Physical Therapist (PT) - Home Health to support care in Richmond, VA. This full-time local contract lasts 14 weeks with an hourly pay range of $60.00 to $66.00 and local travel.

You will perform patient evaluations, develop and update care plans, treat neurological, orthopedic, and neuromuscular conditions in home settings, educate patients and caregivers on mobility and safety, and maintain timely documentation in coordination with care teams.

Qualifications

• Active State PT license is required.
• Current CPR certification is required.
• Valid driver's license and reliable transportation.
• 1–2 years of PT experience in acute care, rehabilitation, or related settings.
• Bachelor's Degree in Physical Therapy from an ACAPT/APTA-approved program.

Responsibilities

• Conduct patient evaluations and develop/update individualized Plans of Care (POC).
• Provide treatment for neurological, orthopedic, and neuromuscular conditions in the home setting.
• Educate patients and caregivers on mobility, transfer techniques, home safety, and therapeutic exercises.
• Complete timely clinical documentation, progress notes, and care coordination activities with physicians and care teams.

Pay Range: $60.00 to $66.00/hour *

Mileage Reimbursement Rate (if applicable): $0.50/ mile

*Pay offered is based on experience, expertise, credentialing, and education.

Requirements

• 1-2 years of PT experience in acute care, rehabilitation, or related settings
• Bachelor's Degree in Physical Therapy from an APTA-approved program
