Job summary

Home Sweet Home Health Agency in the North Phoenix area seeks a licensed Physical Therapist to provide one-on-one in-home care, helping patients regain strength, mobility, and independence.

This role offers flexible scheduling with per diem opportunities, per-visit compensation up to $103,500 annually based on productivity, and a $5,000 sign-on bonus for full-time hires. Visits are documented in EMR and care teams collaborate closely.

Qualifications

• Active Arizona Physical Therapist license.
• CPR certification (AHA).
• Minimum 1 year of clinical experience (home health preferred).
• Strong clinical judgement and time management.
• Reliable transportation, valid driver’s license, and insurance.
• Strong communication and a patient-first approach.

Responsibilities

• Conduct in-home evaluations, start of care, and follow-up visits.
• Develop and implement individualized treatment plans.
• Improve strength, balance, mobility, and functional independence.
• Educate patients and caregivers on safety and equipment use.
• Document visits using WellSky Kinnser EMR.
• Collaborate with physicians, RNs, and the care team.

Your Role: Home Health Physical Therapist
