Job summary

Home Sweet Home Health is seeking a licensed Physical Therapist to deliver in-home care across the Phoenix West Valley region. This role offers per diem, part-time, and full-time options with autonomy and a meaningful patient impact.

You will conduct in-home evaluations, develop individualized plans, and help patients regain strength and independence while collaborating with a dedicated care team.

Qualifications

• Active Arizona Physical Therapist license.
• CPR certification (AHA).
• Minimum 1 year of clinical experience (home health preferred).
• Strong clinical judgment and time management.

Responsibilities

• Conduct in-home evaluations, start of care, and follow-up visits.
• Develop and implement individualized treatment plans.
• Improve strength, balance, mobility, and functional independence.
• Educate patients and caregivers on safety and equipment use.

Compensation: Per Visit | Earn up to $103,500 annually based on productivity

Sign-On Bonus: $5,000

Schedule: Per Diem, Part-Time, and Full time
