Home Sweet Home Health is seeking a Physical Therapist to deliver one-on-one home health care in the Phoenix West Valley region. You will help patients regain strength, mobility, and confidence in daily activities.

Qualifications

• Active Arizona Physical Therapist license.
• CPR certification (AHA).
• Minimum 1 year of clinical experience (home health preferred).
• Strong clinical judgment and time management.
• Reliable transportation, valid driver’s license, and insurance.
• Strong communication and a patient-first approach.

Responsibilities

• Conduct in-home evaluations, start of care, and follow-up visits.
• Develop and implement individualized treatment plans.
• Improve strength, balance, mobility, and functional independence.
• Educate patients and caregivers on safety and equipment use.
• Document visits using EMR.

Location: Phoenix West Valley Region - Glendale, Sun City, Peoria

Compensation: Per Visit | Earn up to $103,500 annually based on productivity

Your Role: Home Health Physical Therapist

You will provide one-on-one care in patients' homes, helping them regain strength, mobility, and confidence in their daily lives.

Key Responsibilities
