Home Sweet Home Health is seeking a Physical Therapist to provide one-on-one, in-home care in the Phoenix West Valley region, including Glendale, Sun City and Peoria. You will help patients regain strength, mobility, and confidence in daily life through personalized home visits.

The role offers per visit compensation with a sign-on bonus and a productive earning potential; flexible per diem, part-time, and full-time schedules are available to fit your lifestyle.

Qualifications

• Active Arizona Physical Therapist license.
• Minimum 1 year of clinical experience in home health preferred.
• CPR certification (AHA).

Responsabilités

• Conduct in-home evaluations, start of care, and follow-up visits
• Develop and implement individualized treatment plans
• Improve strength, balance, mobility, and functional independence
• Educate patients and caregivers on safety and equipment use
• Document visits using WellSky Kinnser EMR
• Collaborate with physicians, RNs, and the care team

Compensation: Per Visit | Earn up to $103,500 annually based on productivity

Sign-On Bonus: $5,000

Schedule: Per Diem, Part-Time, and Full time

Your Role: Home Health Physical Therapist

You will provide one-on-one care in patients' homes, helping them regain strength, mobility, and confidence in their daily lives.

Key Responsibilities
