Job summary

Home Sweet Home Health is seeking a Physical Therapist to provide one-on-one in-home care in the Phoenix West Valley region. You will help patients regain strength, mobility, and confidence through personalized treatment plans and hands-on therapy.

This per diem, part-time, and full-time role offers per-visit compensation with earning potential up to $103,500 annually based on productivity, plus a $5,000 sign-on bonus.

Qualifications

• Active AZ Physical Therapist license.
• Current CPR certification (AHA).
• Minimum 1 year of clinical/home health experience.
• Strong clinical judgment and time management.
• Reliable transportation and valid driver's license.

Responsibilities

• Conduct in-home evaluations and follow-up visits.
• Develop and implement individualized treatment plans.
• Improve strength, balance, mobility, and independence.
• Educate patients and caregivers on safety and equipment use.

Your Role: Home Health Physical Therapist

You will provide one-on-one care in patients' homes, helping them regain strength, mobility, and confidence in their daily lives.

Key Responsibilities

• Conduct in-home evaluations, start of care, and follow-up visits


• Develop and implement individualized treatment plans


• Improve strength, balance, mobility, and functional independence


• Educate patients and caregivers on safety and equipment use


• Document visits using WellSky Kinnser EMR


• Collaborate with physicians, RNs, and the care team

Qualifications

• Active Arizona Physical Therapist license


• CPR certification (AHA)


• Minimum 1 year of clinical experience (home health preferred)


• Strong clinical judgment and time management


• Reliable transportation, valid driver’s license, and insurance


• Strong communication and a patient-first approach
