Home Sweet Home Health is seeking a Physical Therapist to provide in-home, one-on-one care in the Phoenix West Valley region, helping patients regain strength, mobility, and independence in daily life. You will evaluate and treat patients in their homes, develop personalized plans, document visits, and collaborate with physicians and the care team to ensure quality outcomes.

Flexible scheduling includes per diem, part-time, and full-time options with strong earning potential and a $5,000 sign-on

Qualifications

• Active Arizona Physical Therapist license is required.
• CPR certification through AHA is required.
• Minimum 1 year of clinical home health experience is preferred.
• Strong clinical judgment and time management skills are essential.
• Reliable transportation, valid driver's license, and current insurance.
• Excellent communication and patient-first approach are necessary.

Responsibilities

• Conduct in-home evaluations, plan of care initiation, and follow-up visits.
• Develop and implement individualized treatment plans for each patient.
• Improve strength, balance, mobility, and functional independence.
• Educate patients and caregivers on safety, home exercise programs, and equipment use.
• Document visits using the EMR and ensure accurate charting.
• Collaborate with physicians, RNs, and the care team to coordinate care.

Location: Phoenix West Valley Region - Glendale, Sun City, Peoria

Compensation: Per Visit | Earn up to $103,500 annually based on productivity

Sign-On Bonus: $5,000

Schedule: Per Diem, Part-Time, and Full time

Your Role: Home Health Physical Therapist

You will provide one-on-one care in patients' homes, helping them regain strength, mobility, and confidence in their daily lives.

Key Responsibilities

• Conduct in-home evaluations, start of care, and follow-up visits


• Develop and implement individualized treatment plans


• Improve strength, balance, mobility, and functional independence


• Educate patients and caregivers on safety and equipment use


• Document visits using WellSky Kinnser EMR


• Collaborate with physicians, RNs, and the care team
