Job summary

Home Sweet Home Health in Scottsdale, AZ is seeking a licensed Physical Therapist to provide home health care. This role offers flexible scheduling with per-visit compensation and a sign-on bonus for full-time staff.

You will deliver one-on-one care in patients' homes, conduct evaluations, develop plans, and document visits using EMR. Eligible candidates hold an active AZ PT license, CPR, and have at least 1 year of home health experience.

Qualifications

• Active Arizona Physical Therapist license.
• CPR certification (AHA).
• Minimum 1 year of clinical experience (home health preferred).
• Strong clinical judgment and time management.
• Reliable transportation, valid driver’s license, and insurance.
• Strong communication and a patient-first approach.

Responsibilities

• Conduct in-home evaluations, start of care, and follow-up visits.
• Develop and implement individualized treatment plans.
• Improve strength, balance, mobility, and functional independence.
• Educate patients and caregivers on safety and equipment use.
• Document visits using WellSky Kinnser EMR.
