Job summary

CommuniCare Rehab at Burlington Healthcare Center is seeking a Physical Therapist (PT) for a full-time role with a $5,000 sign-on bonus. The therapist will evaluate patients, develop treatment plans, and collaborate with the rehab team to deliver person-centered care.

The position offers opportunities to serve at multiple centers, participate in interdisciplinary meetings, and contribute to high-quality post-acute care for residents with complex needs.

Qualifications

• Must have a Physical Therapy degree from an accredited program.
• Hold a valid or pending State Physical Therapy license.
• Licensure requirements vary by state and facility.

Responsibilities

• Evaluate, review, and screen patient charts and evaluations.
• Develop and implement a treatment plan aligned with patient needs.
• Provide supervision and education to PTAs and rehab staff.
• Collaborate in interdisciplinary meetings and educate families when appropriate.

Qualifications:

• A Bachelor's, Master’s, or Doctorate Degree in Physical Therapy from an Accredited Program.
• Hold a valid or pending State Physical Therapy License

Job Responsibilities include, but are not limited to:

• Evaluate, Review, and Screen patient’s/resident’s medical charts, evaluations under physician order and develop a treatment plan in accordance with patient’s needs.
• Implement treatment plan through direct treatment, as well as education of the plan of care to all team members, including providing necessary supervision for PTAs.
• Attend/Lead interdisciplinary meetings to relay clinical findings and make recommendations to other members of rehab team, as well as conducting staff/family training and education when appropriate.
