Job summary

CommuniCare Rehab is seeking a Physical Therapist (PT) for a full-time role at BridgePort Healthcare Center. The position offers a $10,000 sign-on bonus and responsibilities include evaluating patient charts, developing care plans, and supervising PTAs.

Facilities are part of a larger CommuniCare Family of Companies across several states. Candidates must hold a PT degree from an accredited program and a valid or pending license.

Qualifications

• Bachelor's, Master's, or Doctorate in Physical Therapy from an Accredited Program.
• Hold a valid or pending State Physical Therapy License.

Responsibilities

• Evaluate, Review, and Screen patient’s/resident’s medical charts, evaluations under physician order and develop a treatment plan in accordance with patient’s needs.
• Implement treatment plan through direct treatment, as well as education of the plan of care to all team members, including providing necessary supervision for PTAs.
• Attend/Lead interdisciplinary meetings to relay clinical findings and make recommendations to other members of rehab team, as well as conducting staff/family training and education when appropriate.
• Opportunity to work at multiple centers, allowing you to serve a wide variety of patient needs.
