Victorian Post Acute in San Francisco, CA is hiring a Physical Therapist (PT) for our in-house therapy program. This role offers a competitive starting pay of $60-$65 per hour depending on experience, with opportunities for full-time, part-time or PRN schedules.

We welcome new graduates and provide mentorship, growth opportunities, and a collaborative multidisciplinary team, including PTAs, OTs, SLPs, and nursing staff, to deliver high-quality patient care and meaningful outcomes.

Qualifications

• Current Physical Therapist license in good standing.

Responsibilities

• Evaluate patients and develop individualized treatment plans.

Key Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Provide skilled physical therapy services in accordance with physician orders and care plans.
• Supervise Physical Therapist Assistants (PTAs) in compliance with state practice regulations.
• Collaborate with interdisciplinary team members to coordinate and optimize patient care.
• Participate in care conferences, rehabilitation meetings, and utilization reviews as needed.
• Maintain timely and accurate documentation, including evaluations, treatment notes, progress reports, and discharge summaries.
• Educate patients, families, and caregivers on treatment goals, mobility strategies, safety techniques, and home exercise programs.
• Assist with discharge planning and recommendations for continued care.
• Recommend and coordinate appropriate durable medical equipment (DME).
• Ensure a safe treatment environment by monitoring and reporting equipment maintenance needs.
