Job summary

Pacific Coast Post-Acute in Salinas, CA is seeking a Physical Therapist to join our in-house rehabilitation team on a full-time basis. Responsibilities

  • Perform comprehensive physical therapy evaluations and develop individualized treatment plans based on resident goals and functional needs.
  • Provide skilled interventions, including gait training, therapeutic exercise, balance training, manual therapy, and neuromuscular re‑education.
  • Monitor resident progress and modify treatment plans as appropriate to maximize functional outcomes.
  • Guide residents through their rehabilitation journey, from acute recovery through safe discharge and community reintegration.
  • Collaborate with the interdisciplinary care team to coordinate effective, resident‑centered care.
  • Supervise Physical Therapist Assistants (PTAs) in accordance with applicable California practice requirements.
  • Maintain accurate and timely documentation, including evaluations, daily treatment notes, progress reports, recertifications, and discharge summaries.
  • Participate in care conferences, clinical meetings, and quality improvement initiatives.
  • Communicate effectively with residents, families, caregivers, and other members of the healthcare team.
