Santa Cruz Post Acute is seeking a dedicated Physical Therapist (PT) to join our rehabilitation team, evaluating residents and delivering skilled therapy to improve strength, mobility, and independence. Full-time, part-time, and PRN opportunities are available with starting pay $50 per hour DOE plus benefits; you’ll collaborate with nurses, therapists, physicians, and families.

Applicants must be CA-licensed PT with an accredited program; SNF experience is preferred but not required; strong

Qualifications

• Current Physical Therapist license in California.
• Graduate of an accredited Physical Therapy program.
• Strong communication, documentation, and time‑management skills.
• SNF experience preferred but not required—new graduates are welcome!

Responsibilities

• Conduct comprehensive evaluations and develop individualized treatment plans for residents with physical and functional impairments.
• Provide therapeutic interventions including gait training, strengthening, balance activities, pain management, and mobility training.
• Document evaluations, daily notes, progress reports, and discharge summaries in accordance with regulatory and facility standards.
• Collaborate closely with nursing staff, therapy team, physicians, and families to support resident goals.
• Educate residents and caregivers on exercise programs, safety techniques, and mobility strategies.
• Participate in interdisciplinary care conferences and contribute to overall care planning.
• Maintain compliance with state, federal, and professional practice regulations.

Santa Cruz Post Acute is seeking a dedicated and compassionate Physical Therapist (PT) to join our skilled rehabilitation team. We deliver patient‑centered care in a supportive, collaborative setting where your expertise truly makes a difference. Full‑time, part‑time, and PRN opportunities are available.

Position Summary
